For this peer learning session, our discussion focused on the case study brief, Advancing Coordinated Care during a Pandemic, based on Hudson Headwaters Health Network’s (HHHN) approach.
When the COVID-19 pandemic began to spread throughout nursing homes in 2020, care teams at HHHN were optimally positioned to step in and provide palliative care services for individuals with serious illness and their families. With over 15 years’ experience delivering community-based primary and palliative care, HHHN established a clear chain of authority, allocated appropriate staffing to address infection surge and control, and prioritized communication to coordinate and deliver excellent and compassionate care to the most vulnerable members of their communities.
