Supporting FQHCs in Care Management

This peer learning session discussed the Institute’s recently published case study brief on Medical Home Network and their efforts to support federally qualified health centers (FQHCs) in the development of practice-level care management capabilities. Many of MHN’s patients are from historically marginalized groups that achieved access to healthcare post-civil rights era through FQHCs but are now aging into Medicare with different needs requiring care delivery innovation.
